It is possible that the wrong amount of protection lacquer has been applied to the brake drums. This could allow settlement of the road wheel during use. This can cause the loosening of the securing bolts which could result in the detachment of the road wheel.

Remedy

Recall all affected axles inspect and if require replace the brake drums.
